The index list type is specified on the main interface:
Indices can be listed either in a column (vertically) or in a row (horizontally).
Most of the examples in this help guide have shown data indices listed in a column. If indices (in this case, ZIP+4 codes) are listed across a row, specify in the settings interface that "Indices are listed in a row" and the resulting worksheet will look like this:
